How to Get a Tooth Tattoo

1. Print out the Tooth Tattoo Request Form (Rx), and take it to your dentist. You'll need the artwork for your tooth. Remember that the tooth is very small, so a simple design is better.
2. Your dentist will prepare your tooth and take an impression (could be digital). He/she sends the form, along with the specifications for the crown, bridge, or implant tooth to Westbrook Dental Studio. If any quesions, your dentist can call 708-862-5660.
3. After the new tooth with the tooth tattoo is made, it will be shipped to the dentist.
4. Return to your dentist. The dentist will cement your new tooth with the dental art.
